Event: Riley Rotkiewicz Graduation Party

Date/Time: Saturday, May 23, 2026 at 12:00 PM

Location: 615 Rice Street E, Wayzata, MN 55391

No Alcohol: No alcohol will be served or allowed. Violation may result in removal.

Activities: The event includes a bounce house, water slide, and other recreational play. These activities involve inherent risks including slips, falls, collisions with other children, bumps, bruises, sprains, fractures, or more serious injury.

By signing below, the parent/guardian acknowledges and agrees:

1. I have read and understand the risks. I voluntarily assume all risks of injury to my child(ren) and myself while attending or participating.

2. I release and hold harmless Stephen Rotkiewicz, hosts, family members, and property owners from any and all claims for injury, damage, or loss arising from ordinary negligence or inherent risks of the activities, to the fullest extent permitted by Minnesota law. This release does not apply to gross negligence, reckless conduct, or intentional acts.

3. I agree to indemnify and defend the hosts against any claims brought by or on behalf of my child(ren) or guests under my supervision.

4. I will supervise my child(ren) at all times. Hosts are not responsible for child supervision.

5. Media Consent: I grant Stephen Rotkiewicz and RocklobsterCraft Productions irrevocable permission to photograph, film, and use any images/audio of my child(ren) and family for promotional, documentary, or entertainment purposes without compensation. I waive any rights of privacy or publicity claims.
